Intertrigo: Description, Causes and Risk Factors: Intertrigo is an inflammatory condition of the skin folds induced or exacerbated by heat, moisture, and friction eroding the opposing skin surfaces, which usually located in the inner thighs, armpits, and underside of the breasts or belly. It is red and raw looking and may itch, ooze or be a little sore. IntertrigoIntertrigo is common, especially in hot humid climates. The condition is a frequent complication of diabetes or in those who are overweight. It also affects most infants as a component of diaper dermatitis. It can involve secondary colonization, most commonly candidal, but may also involve bacterial, fungal, or viral pathogens. The affected areas of skin are usually pink to brown. If the skin is particularly moist, it may begin to break down. In severe cases, there may be a foul odor. Intertrigo is also commonly seen in people with advanced age and subsequent reduced immunity. It may also be seen in people who must stay in bed or who wear medical devices, such as artificial limbs, splints, and braces, which may trap moisture against the skin. Symptoms: There are many signs and symptoms which help you identify intertrigo. It is a skin disorder on the outer layer of the skin and manifests itself in physical form. The most common sign of intertrigo is constant itching and severe temptation to scratch the skin surface and it is often relieved by scratching in most cases. Itching occurs under the skin fold and it is sometimes not approachable place. The area of the skin around rashes becomes raw and red. As the intertrigo infection develops, skin lesions become severely inflamed and increasingly irritated. Diagnosis: The diagnosis is based on the presence of pruritus, erythema, tenderness, or increased sensitivity to the affected skin folds. Clinical appearance is usually sufficient for a diagnosis of intertrigo. Tests may include: A skin scraping and KOH examination may also be done to rule out a fungal infection.
  • A Wood's lamp may be done to rule out a bacterial infection called erythrasma.
  • Some time skin biopsy is also needed to rule other conditions.
Treatment: Treatment should have the focus of minimizing moisture and friction. Drying the area thoroughly after showering and keeping it open to air when possible are important in relieving symptoms and preventing recurrence. Keeping the folds cool and dry and eliminating any friction to the area is vital to improving this condition, absorbent powders, compresses with Burow's solution 1:40 dilute vinegar or wet tea bags often are and effective treatment, especially if followed by fanning or cool blow-drying. Candidal infections need to be treated with topical antifungal creams, lotions, or solutions. Bacterial causes may include both topical and oral antibiotic regimens. Low-dose topical steroid therapy may also be useful in decreasing inflammation and associated symptoms. Commercially available barrier pastes sold for diaper dermatitis (e.g., Desitin) can help, as can absorbent diapers. If the person suffering with intertrigo is a diabetic patient, then he should control his sugar level. Excessive sweating can be controlled with respective antiperspirant.
